Screen for Obstructive Sleep Apnea with apneABP: Holter ABPM with SpO2 & Actigraph
Undiagnosed obstructive sleep apnea syndrome (OSAS) improves the risk of sleep-related traffic accidents. Meditech apneABP combined Holter ABPM, pulse oximeter (SpO2) and actigraph can be considered a small ambulatory sleep lab which supports the convenient and cost-effective diagnosis of obstructive sleep apnea, a major contributing factor in the development of hypertension. Patients with suspected obstructive sleep apnea syndrome are often referred to polysomnography in a sleep lab, but the test is expensive and scarcely available. Also, in around 30% of all patients that undergo a polysomnography, no apnea is detected. Meditech apneABP combined Holter ABPM, pulse oximeter, and actigraph is a convenient and affordable screening solution to exclude sleep apnea in primary care.

Holter ABPM performed on patients with obstructive sleep apnea usually shows elevated night-time blood pressure, typically of diastolic BP values. The normal night-time dip in the Holter ABPM profile is usually narrowed, missing, or might even be reversed: the majority of patients with obstructive sleep apnea are non-dippers. As sleep apnea episodes trigger general alarm reactions, such episodes are followed by elevation of blood pressure and heart rate, therefore, night-time blood pressure variability is usually seriously elevated.

Pulse oximetry, monitoring blood oxygen saturation (SpO2) levels, performed on patients with obstructive sleep apnea will show reduced blood oxygen levels during apneic episodes. In severe obstructive sleep apnea syndrome, blood oxygen levels will drop to as low as 70%.

Actigraphy (monitoring of body position and physical activity/motion levels) performed on patients with obstructive sleep apnea usually shows higher than normal levels of activity, partly due to loss of sleep quality and a series of partial or full awakenings, and partly due to the typically present nycturia which causes patients to leave bed to urinate several times during the night. Actigraphy is helpful in differentiating early-morning apnea, hypopnea episodes caused by sleep apnea from those caused by chronic obstructive pulmonary disease and/or asthma.

A growing amount of evidence suggests that obstructive sleep apnea is the most common cause of secondary hypertension, but an estimated 80-90 percent of cases are still undiagnosed. However, eliminating snoring and the occurrences of apnea-hypopnea episodes have a significant effect on general functioning, which would contribute to the decrease of motor vehicle crashes, the most common adverse outcome of excessive daytime sleepiness.
